Who are we? Children's Community Nursing Team
The Children’s Community Nursing Team (CCNT) was established in 1995. Since then, the team has grown in response to the need to provide ‘care closer to home’ for children and young people. The team is part of the Integrated approach to preventing and avoiding hospital admission here at Tameside and Glossop Integrated Care NHS Foundation Trust, following the ambulatory care model. The team has built up strong links with the local GP community, working together to look after children closer to home.
Where are we based?
The CCNT is based within the Children’s Outpatients’ Department Tameside Hospital and provides nursing care for all children 0-16 years in the Tameside and Glossop area. The team can be contacted on 0161 922 5251 (voicemail available).
The CCNT has worked hard to be responsive to the needs of the local community and has developed a co-ordinator role for easy access to a senior nurse between the hours of 8am and 8pm Monday to Friday. The team run nurse led clinics 7 days a week offering a service to non-acutely ill children for such things as wound checks, burns dressings, BP recordings and long-term condition management. The team work in collaboration with other specialist teams to support children and young people requiring end of life care at home.
Our Team
The CCNT is comprised of experienced paediatric nurses who care for children in the home/community setting with a wide range of clinical specialties, medical, surgical, oncology, cardiology and neonatal and are supported by our admin team.
The CCNT work from 8am to 8pm 365 days of the year and are able to accept referrals from all members of the multidisciplinary team. The team will accept self-referrals from parents for wound care only. Referrals are received by email: ccnttameside
